Plas Ffynnon is a safe surgery, committed to tackling the barriers that many migrants have in accessing healthcare. Anyone in our area can register with us. It’s free to register. You do not need proof of address or immigration status, ID or an NHS number. Immigration status or nationality don’t matter – reception won’t ask for immigration documents and won’t share your information with the Home Office unless serious crime is involved. We do not want anyone in our community to be excluded.

To register, please check you are within our Practice boundary (see map of Practice area). The Reception Staff can assist you to complete the necessary forms.
To register at the surgery, you will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. These forms are available at reception or can be downloaded below, printed out, filled in and returned:
